Service coordination is facilitated by Primary Care Partnerships where agencies come together to agree on how they will coordinate their services so that consumers experience a health system that works together.
Service coordination aims to place consumers at the centre of service delivery – ensuring that they have timely access to the services they need, opportunities for early intervention and health promotion and improved health outcomes.
Supporting Service Providers to work together to align practices, processes and systems
The Service Coordination Online Learning Module is an e-learning tool to support the practice of Service Coordination in Victoria. Organisations are encouraged to use this module to orientate staff to Service Coordination practice within their organisation and use it for professional development for existing staff. The Learning Journal within this module can be used for Staff Supervision and is a great tool for staff to reflect on service coordination practice in their own workplace.
